- Abstract
This letter to the editor responds to a narrative review on the early diagnosis of herpes zoster neuralgia. The author raises concerns about the inconsistent and confusing wording used in the review, particularly regarding the distinction between local pain before or during shingles and neuropathic pain associated with post zoster neuralgia (PZN). The author also questions the goals and terminology used in the review, as well as the possibility of diagnosing neuralgia before the onset of symptoms. Additionally, the author disagrees with the statement that cerebrospinal fluid (CSF) testing for varicella zoster virus (VZV) is not suitable for ordinary patients with shingles, arguing that lumbar puncture should be performed in all patients with or without shingles if there is suspicion of central nervous system (CNS) involvement. The author concludes by emphasizing the importance of distinguishing between pain that occurs with the rash and pain that develops after the resolution of shingles.
