- Abstract
A retrospective analysis was conducted on patients with pT1b melanoma to evaluate the role of sentinel lymph node biopsy (SLNB) in their management. The study found that the incidence of a positive SLNB in pT1b melanomas is low, but the implications for staging, follow-up, and access to adjuvant therapy are significant. The analysis revealed that some patients with positive SLNB did not meet the high-risk criteria outlined in the NICE 2022 guidelines, suggesting the need for alternative risk stratification methods. The study also highlighted the importance of a mitotic index of ≥ 2 as a predictive risk factor for a positive SLNB. These findings may inform discussions on offering SLNB to patients with pT1b melanoma with higher melanoma risk prediction scores, even in the absence of other risk factors.
