- Abstract
Defination: Any untoward event which occurs during or after transfusion of blood or blood products. Materials and Methods: A four year study (2002-2005) of 2125 units of whole blood at C.H.NAISARAI. Show 40 number of incident of various types of T/R which has been investigated / analyzed to prevent further recurrence. Object: • Supply of needed functional components. • No immediate / delayed ill effect. • Monitoring and quick note of adverse effect. • Immediate action to investigate and prompt action to stop. • Investigate NOT for passing blame BUT note cause and learn for further Analysis: • The rate of T/R at C.H.N 0.02% which are far below then national level 6.5% due to high SOP of blood bank • Majority 0.01% due to fault of staff /doctor not allowing blood to come to room temperature. • Sub - group incompatibility / rechecking of grouping would avoid other type of T/R • Volume overload could be avoided by component / packed cell transfusion Protocol: • Stop transfusion keep I.V line open • Preserve residual blood. • Monitor pulse, temp. BY and urine out put. • Institute medical treatment. • Inform lab in prescribed format. Conclusion: • Through monitoring • Adhere to SOP. • Lab investigation • Haemovigilance by hosp. transfusion committee.
