Demodex blepharitis, caused by Demodex mites that reside on or between eyelashes, can lead to inflammation and hypersensitivity reactions. While there is no standard treatment for these mites, lotilaner ophthalmic solution 0.25% has shown promising results in reducing the number of mites and collarettes. Other treatment options include hypochlorous acid solutions, tea tree oil, and a mixture of coconut oil, tea tree oil, and Manuka honey. Cleaning options such as microblepharoexfoliation and ZEST are also effective. Clinicians should discuss makeup use and eyelash extensions with patients, and intense pulsed light therapy and ivermectin cream may be considered for more severe cases. Patient compliance is crucial for successful treatment.
