Nocardiosis in rheumatic patients: Case series and review of literature.

- Abstract
This case indicates that nocardiosis can aggravate the gravity of rheumatoid arthritis, so potential infection such as nocardiosis may need to be considered when treating difficult-to-treat arthritis. In addition to these, due to the high infectious tendency to CNS of nocardiosis, we suggest that patients with pulmonary nocardiosis or who are immunocompromised may need brain MRI examination even when typical CNS symptoms were not shown. As for treatment, SMZ-TMP is the optimal medication against nocardiosis, and a combination of drugs is required for patients with SMZ-TMP resistance or severe infections. The use of corticosteroids, immunosuppressants, and/or biologics in patients with rheumatic disease may suppress their immune system, and therefore induce opportunistic infections including nocardiosis.
