- Abstract
The pain in the newborn (NB) was not a matter of concern for clinicians and researchers for a long time, until 1980. There was a great reluctance to use of opioids effectively, partly due to the fear of side effects such as susceptibility to respiratory depression and addiction; reason why the pain in NB was forgotten and not treated. A developed organism has the mechanisms for regulating transmission of a painful message, both at spinal and supraspinal level. There is a controversy about physiology of pain in the premature and it is known supraspinal control is not functional in the NB. The scales of pain based on changes in behaviour most commonly used are: NFXS, IBCS, NIPS, LIDS, CHEOPS, NAPI. Other scales combined with behavioral or physiological changes are: CRIES, PAT, PIPP, SUN and CONFORT scores. There are many painful procedures that produce recurring pain in the NB during diagnostics or therapeutic as blood taking, placing intravenous catheters, postural changes, removal of adhesive material and explorations; without forgetting the pathological conditions that can cause pain at any time. There is no scientific evidence to support fears about the danger of using a strong analgesic in newborns. Lack of recognition of the need for an appropriate and effective treatment has influenced the paucity of clinical studies regarding the use of painkillers and new techniques to avoid pain in NB.
