Dear Editor, We report a case of refractory status epilepticus who developed bilateral pneumothorax and tracheoesophageal fistula following percutaneous dilatational tracheostomy (PDT). The modified Seldinger technique of PDT under bronchoscopic guidance reduces complication due to blind placement of tracheostomy tube.
