The article discusses the importance of patient-centered medical homes (PCMHs) to health educators. Topics include an overview of the PCMH model, which is an approach to providing comprehensive care in a primary care setting, and the enhancement of primary care delivery as a result of health educators involvement in the implementation of the PCMH model.
