ESTUDIO DEMOGRÁFICO DE PACIENTES CON MIASTENIA GRAVIS DEL INSTITUTO NACIONAL DE NEUROLOGÍA Y NEUROCIRUGÍA.

- Abstract
Objetivo: Descripción del perfil demográfico y clínico de los pacientes con Miastenia Gravis Autoinmune del Instituto Nacional de Neurología y Neurocirugía. Antecedentes: Miastenia Gravis Autoinmune (MGA) es la enfermedad más común de la unión neuromuscular (UNM) caracterizada por debilidad muscular y fatiga, mediada por anticuerpos contra la membrana postsináptica y de la UNM. La afección de la transmisión neuromuscular conduce a debilidad y fatiga; la fluctuación en la severidad de la debilidad muscular es típica, no todos los músculos están afectados y puede ser asimétrica. MGA tiene una incidencia anual de 8-10/1,000,000 individuos y una prevalencia 150-250 casos por millón de habitantes con una presentación bimodal, el 10% se relaciona a timoma. México cuenta con un estudio epidemiológico en 2010 donde se refieren cifras de egreso anual con diagnóstico de Miastenia en los diversos sistemas de salud en donde se reporta que, de 4,252,312 adultos egresados solo 587 correspondían a miastenia 0.01%. Métodos: Estudio transversal ambispectivo. Se tomó una muestra por conveniencia de 192 sujetos, se incluyeron pacientes mayores de 18 años que acuden a la Clínica de nervio y Músculo del Instituto Nacional de Neurología y Neurocirugía durante el periodo comprendido entre enero 2016 a julio 2023. Resultados: Se incluyeron 192 pacientes, de los cuales el 54.7% eran de género femenino, con una edad promedio de 49.22 años. La mayoría de los pacientes residen en la zona centro del país, específicamente 170 pacientes que representan el 88.5% de toda la muestra. El 45.3% debutaron con una presentación ocular, mientras que el 41.1% lo hizo de forma generalizada. El 6.8% presentó crisis miasténica asociada y el 5.7% mostró síntomas bulbares. La MGA de inicio temprano se observó en el 59.4% de los casos, y solo el 12% informó comorbilidades autoinmunes, con un 10.9% de pacientes con antecedentes heredofamiliares de autoinmunidad. De los pacientes, 161 resultaron ser seropositivos a anticuerpos antirreceptores de acetilcolina, lo que representa el 83.9% de la muestra. Además, 142 pacientes se sometieron a estimulación repetitiva, de los cuales 118 obtuvieron un resultado positivo. Conclusiones: Hacen falta mayor seguimiento a nuestra población de pacientes con MGA y de esta forma conocer su evolución en nuestro país. El presente estudio muestra la concordancia en picos de edad y manifestaciones clínicas de la enfermedad con respecto a otras bases de datos publicadas en otras regiones del mundo. El Instituto Nacional de Neurología y Neurocirugía inicia esta base de datos con la intención de mejorar la atención y seguimiento de los pacientes con MGA.
