Factors related to poor control of hypertension in the hypertension clinic.

- Abstract
Objective: A study was conducted at the hypertension clinic, Hospital Universiti Sains Malaysia to assess the proportion of patients who achieved their blood pressure targets and to determine the factors associated with uncontrolled hypertension. Design: A cross-sectional study. Materials and Methods: A total of 245 patients who fulfilled the criteria and gave written consent were recruited into this study. Patients' blood pressure (BP), smoking history, weight, height, urine dipstick for protein, fasting lipid profile, serum uric acid, serum creatinine and fasting plasma glucose were obtained. The body mass index (BMI) and estimated glomerular filtration rate (eGFR) of each patient were calculated. Hypertension was defined as controlled if the BP was < 140/90 mmHg or < 130/80 mmHg in patients with diabetes mellitus or chronic kidney disease. Results: Thirty four percent of patients achieved their BP targets. Multiple logistic regression analysis revealed significant independent factors related to uncontrolled hypertension were gender, race, smoking history, level of eGFR and diabetes mellitus. The non-Malay patients were less likely to have uncontrolled hyper-tension compared to Malay patients (p = 0.031, OR = 0.43, 95%CI = 0.20, 0.93). Female patients (p = 0.001, OR = 4.68, 95%CI = 1.83, 11.98), smokers and ex-smokers (p < 0.001, OR = 8.58, 95%CI = 3.60, 20.45), eGFR (p < 0.001, OR = 0.91, 95%CI = 0.87, 0.95) and diabetes mellitus (p = 0.001, OR = 9.44, 95%CI = 2.43, 36.70) were significantly associated with uncontrolled hypertension. Conclusion: These results indicated that there was a high prevalence of uncontrolled hypertension at our hypertension clinic and the associated risk factors were female gender, Malay race, a history of smoking, low eGFR and presence of diabetes mellitus.
