Endometrial extragonadal yolk sac tumour-like carcinoma in a postmenopausal patient: A case report.

- Abstract
Background: Primary endometrial yolk sac tumour (YST) in a postmenopausal woman is extremely rare and may be misdiagnosed. Case: A 75-year-old female presented with a history of painless, irregular vaginal bleeding. An endometrial biopsy was reported as high-grade adenocarcinoma of the endometrium. A total laparoscopic hysterectomy with bilateral salpingo-oophorectomy was performed. During the surgical procedure frozen sections of right paratubal tissue and a mesenteric nodule were positive for carcinoma. Final pathology demonstrated an endometrial YST, International Federation of Gynecology and Obstetrics (FIGO) stage IV. Postoperative alpha-fetoprotein was 6930 ng/mL. After 3 cycles of bleomycin, etoposide and cisplatin-based adjuvant chemotherapy, the patient developed a pulmonary embolism and chemotherapy was stopped. Conclusion: Endometrial YSTs are extremely uncommon tumours, especially in postmenopausal women. Oncological management needs to be carefully tailored in this fragile population.
