I Editor i We read with interest the study by Boyd I et al i .[1] which found an increase in mortality in colorectal and upper gastrointestinal (GI) emergency laparotomy performed by non-specialist surgeons. In our institution, all emergency laparotomies during office hours are handled by a dedicated Emergency Surgery and Acute Trauma (ESAT) service[2]. Furthermore, in the era of geriatric surgery[5], the need to deliver appropriate and robust perioperative care to the elderly undergoing emergency laparotomy is increasingly important.
