Trastornos somatomorfos: un nuevo enfoque para el DSM-V.

- Abstract
El DSM-III introdujó la hipotética categoría diagnóstica de los trastorno somatomorfos para ubicar los síntomas somáticos que "no se explicaban por una afección médica general. " Aunque en el DSM-IV se ha conservado y ampliado esta categoría diagnóstica, los trastornos somatomorfos han sido objeto de críticas continuas por parte de médicos y pacientes. El dilatado período de preparación del DSM-V ofrece una oportunidad única para reconsiderar la categoría de los trastornos somatomorfos. Método: La exploración de los diversos objetivos de una clasificación diagnóstica comporta que los autores no sólo deben evaluar los problemas teóricos y prácticos relacionados con esta categoría, sino también conciliarla con la clasificación médica descriptiva paralela de los síntomas y síndromes funcionales. Resultados: Las categorías actuales de los trastornos somatomorfos deben ser modificadas. Los autores abogan por la opción radical de eliminar estas categorías. Los diagnósticos incluidos actualmente entre los trastornos somatomorfos podrían redistribuirse en otras agrupaciones, y los trastornos que actualmente solo se definen por los síntomas somáticos podrían situarse en el Eje III como "síntomas y síndromes somáticos funcionales. " Más práctico sería situar los "factores psicológicos que afectan a una condición médica" en el Eje I. Los autores sugieren complementar el diagnóstico de síntomas somáticos funcionales con una formulación multiaxial. Conclusiones: Los autores abogan por una clasificación de los síntomas somáticos en el DSM-V que sea compatible con la que se utiliza en medicina general y que ofrezca una oportunidad adicional para investigar la etiología y el tratamiento sintomático y para que la psiquiatría se integre mejor en la práctica médica general.
