- Abstract
Pre-operative and peroperative discrimination between uniglandular disease and multiple gland disease remains the key problem in parathyroid surgery. Since the introduction of localising methods such as technetium-99m-sestamibi scanning and ultrasonography, minimally invasive surgery for primary hyperparathyroidism can be performed in case of a single hyperfunctioning adenoma. Quick parathyroid hormone (qPTH) measurement is used to determine if adequate parathyroid tissue has been removed preoperatively. A reduction of qPTH value more than 50%, 5 min after removal of hyperfunctioning parathyroid tissue is generally assumed to indicate complete parathyroidectomy. However, false positive results have been reported and therefore the specificity of this technique demands improvement.
