Case report: high polar bodies aneuploidy rate in a patient with severe ovarian hyperstimulation syndrome.

- Abstract
Aim: Stimulation protocols used for in vitro fertilization (IVF) may be partially responsible for an increased risk of aneuploidy in the maturing oocytes and their first and second polar bodies. This risk may be attributed both to the high levels of gonadotropins used as well as individually increased susceptibility to these hormones that may be responsible for higher risk of ovarian hyperstimulation syndrome (OHSS). Method: We report a case of first IVF cycle in a 29 years old female, who is a heterozygote for Asn680Ser polymorphism in the follicle stimulating hormone (FSH) receptor gene, that increases its sensitivity to FSH. Short stimulation protocol with antagonists resulted in 40 cumulus-oocyte complexes and 35 mature oocytes that were fertilized by intracytoplasmic injection. Simultaneous biopsy of both polar bodies was performed in 22 zygotes followed by fluorescence in situ hybridization (FISH) with probes for chromosomes 13, 16, 18, 21, 22 and X. Results: At least partial results were obtained for 95% of zygotes with 29% normal and 66% abnormal findings. In one case we have found a rescue of first meiotic error by a second meiotic error resulting in a normal final status. The first polar bodies (PB1) were successfully analyzed in 95% of zygotes, a normal result was found in 38% and abnormal in 62% cases. The second polar bodies (PB2) were obtained and successfully analyzed in 73% of zygotes with 62% being normal and 38% abnormal. In 6 cases we were able to reanalyze the embryos that were not selected for transfer by analyzing individual blastomeres. In 2 embryos from zygotes with abnormal PB1 and normal PB2 the results were normal. Remaining 4 embryos were mosaic. Conclusion: This is a first case report, that shows a possible relationship between the FSH receptor gene polymorphisms, the risk of OHSS and increased aneuploidy rate.
