- Abstract
Wstęp: Celem pracy jest określenie przydatności zastosowania ciągłego monitorowania saturacji mózgu (rSO2, regionalcerebraloxygen saturation) w wykrywaniu niedokrwienia centralnego układu nerwowego w trakcie endarteriektomii tętnicy szyjnej. Materiał i metody: Operowano 44 chorych z powodu zwężenia tętnicy szyjnej wewnętrznej w znieczuleniu miejscowym. Jednoczasowo monitorowano poziom saturacji zarówno po stronie operowanej, jak i przeciwnej. Do pomiaru oxymetrii śródmózgowej użyto aparatu INVOS-4100 (Somanetics, Inc. Troy, MI). Wyniki: Po stronie operowanej po zamknięciu tętnicy szyjnej wewnętrznej ICA następował spadek saturacji z 65,1% ± 8,1% do 58,2% ±10,7% (p < 0,005). Po stronie przeciwnej po zamknięciu ICA następował niewielki wzrost saturacji z 65,7 ± 9,2% do 66,1 ± 10,2% (p = 0,1). U 5 chorych (11,4%) po zamknięciu ICA pojawiły się objawy neurologiczne. W 4 przypadkach konieczne było założenie shuntu. W tej grupie chorych spadek rSO2 wahał się średnio 19,2 ± 14%. W grupie chorych bez objawów neurologicznych (n = 39) w trakcie zamknięcia ICA spadek rSO2 wynosił 9,7 ± 10,3% (p = 0,5). W przebiegu pooperacyjnym nie było przypadków udaru lub zgonu. Przy spadku saturacji powyżej 20% czułość badania została oceniona na poziomie 20%, a swoistość na 97,5% natomiast wartość predykcyjna ujemna na poziomie 90,7%. Wnioski: Ciągły pomiar rSO2 jest prostym i nieinwazyjnym sposobem monitorowania chorego w trakcie CEA. Po zamknięciu ICA dochodzi do istotnych zmian w poziomie rSO2. Nie można określić progu spadku rSO2, poniżej którego konieczne jest użycie shuntu ze względu na niską czułość i swoistość badania.
