- Abstract
Typical and atypical antipsychotics, mood stabilizers and antidepressants have been used in the treatment of bipolar disorder. Olanzapine, an atypical antipsychotic drug with serotonin-dopamine antagonism, has been used in the treatment of schizophrenia and bipolar disorder. The long-term olanzapine treatment in two adolescent cases with bipolar disorder is described. One of the cases was followed-up for 4 years (from 16 years of age, two years of olanzapine treatment) and the other case was treated for 10 years (from 14 years of age, one year with olanzapine treatment). Both cases had previously used other antipsychotics and mood stabilizers. At follow-up, the first patient was on valproic acid 1000mg/d and olanzapine 10 mg/d, and the other patient was on lithium 1200mg/d and olanzapine 10 mg/d. Neither of the patients experienced a manic episode for almost a year with these regimens. However, olanzapine was discontinued by the new psychiatrist in the first case and by the patient in the second case. Both cases had manic episodes after cessation of olanzapine. Ten mg/d olanzapine was resumed in both patients, but 20 and 30mg/d olanzapine respectively were necessary in order to achieve remission. The relapse of manic symptoms in both cases after cessation of olanzapine despite continued use of mood stabilizers suggested that olanzapine might be effective in prevention of manic episodes by itself.
