- Abstract
The article discusses the importance of patient navigation to address disparities in palliative care. The roles of a patient navigator vary across programs but usually include some degree of case management, patient education, social work, and advocacy. The first program of patient navigation aimed at breast cancer was designed by Dr. Harold Freeman in 1995.
