- Abstract
Venous ulcer occurs in 1% of adult population and is associated with chronic disability, diminished quality of life and high health care costs. The guidelines has been prepared by a multidisciplinary group of experts based on evidence-based data as well as personal experience. The document presents basic data on epidemiology, risk factors, pathophysiology, symptoms and clinical approach with differential diagnosis of venous ulcers. The etiology of the ulcer should be identified with appropriate management. Effective methods accelerating healing include: compression therapy, medical treatment with pentoxyphylline and sulodexide, surgical treatment with saphenous ablation by chemical, thermal or mechanical means. Local treatment of venous ulcer should follow TIME (tissue debridement, infection and inflammation control, moisture balance and epidermization stimulation). Healing is accelerated with application of specialized dressings. In special situations systemic administration of antibiotics, analgesics or nutritional treatment may be indicated. Secondary prevention involving life style modification is necessary. Patients' care should be provided by a multidisciplinary team including qualified nurse.
