- Abstract
Macrolides have been progressively used in many chronic pulmonary diseases, including Chronic Obstructive Pulmonary Disease (COPD), mainly because of their broad spectrum antibiotic activity and, more recently, their anti-inflammatory and immunomodulatory properties. Purpose of this review is to analyze the scientific evidence regarding the use of long-term low-dose macrolides in COPD, focusing both on the possible mechanisms of action, in particular the immunomodulatory effect, and the major clinical trials currently available. In addition, new studies have explored the role of macrolides on pulmonary microbioma, in fact, while the anti-inflammatory and immunomodulatory effects seem to act on the prevention of COPD exacerbations, the antibiotic activity of macrolides, especially if applied on the long term, could favor the selection of resistant bacteria and reduce the diversity of microbial flora. In this scenario, new molecules belonging to the macrolide family but designed to minimize the antibiotic effect and maximize the immunomodulatory action have been developed and studied.
