- Abstract
This article discusses a case study of a 75-year-old man who presented with lethargy and diarrhea. His admission digoxin level was falsely elevated, leading to consideration of digoxin-specific antibody fragments administration. However, further analysis using alternate digoxin assays revealed that the patient was not taking digoxin but was instead taking enzalutamide for prostate cancer. The article highlights the potential for false positive digoxin levels on specific assays in patients taking enzalutamide and emphasizes the importance of understanding laboratory assays to avoid misdiagnosis and potential mismanagement. The use of quadrupole time-of-flight mass spectrometry analysis is also discussed as a new technology that can confirm the presence of enzalutamide and absence of digoxin.
