- Abstract
Here we present the case of 78 year old male admitted to cardiology ward due to right heart failure symptoms and very severe tricuspid regurgitation. It was a pacemaker dependent patient with complete atrioventricular block, implanted in 2013 and upgraded to CRTP in 2023. Despite positive CRT response on left ventricular function, echocardiography exam revealed progression of tricuspid regurgitation due to complex mechanism including propping upward the leaf- lets by RV lead and excessive LV lead loop. 3D echocardiography guided decision was made to perform TLE and change leads. To minimalize negative influence on tricuspid valve 4F lumenless Medtronic 3830 RV lead was used followed by LV change. We observed reduction of tricuspid regurgitation from severe to moderate and right ventricular function improvement resulting in NYHA class symptoms reduction from III to II, improvement in 6MWT from 120 M to 420 M during early 3-months follow up. CONCLUSIONS However TLE for improvement of tricuspid valve function remains controversial indication, it may play a role and reduce right heart failure symptoms and tricuspd regurgitation in highly selected cases identified by novel echocardiographic techniques (3D echo). Rigorous lead positioning and selection is of crucial role during implant to minimize the risk for LDTVD development.
