Introduction and Objective: Although PCN is indicated for urinary diversion in obstructed system with infection and obstructed symptom with deteriorating renal function. we can use it to save kidney, from nephrectomy based on renogram study at initial presentation in paediatic age group. Materials and Methods: A total 24 patients were studied between June 2003 to June 2007. Patients presenting with gross hydronephrosis with split function < 10% included in this study. Patients with infection and split function >10% and age more than 15 years were excluded from the study. PCN was done in all cases to see improvement in renal function after 4 weeks. Follow up with renogram was done after 4 weeks. Result: In this study children between 3-6 year were most benefited from PCN. Out of 12, 10 patients improved with PCN i.e. success rate was 83% while in age group of 6-9 years, one patient out of 4 improved. No significant improvement in renal function was seen in patient of 9 year and above age group. Conclusion: The role of PCN in poorly functioning or non functioning grossly hydronephrotic kidney due to PUJO is unpredictable, PCN can be recommended in children below 6 years with very low GFR and gross hydronephrosis and waiting for at least 4 weeks to see improvement in GFR before going for nephrectomy.
