- Abstract
This letter, published in the Journal of Neurology, describes a case of subacute transverse myelitis in a 53-year-old man who had previously experienced a cryptogenic ischemic stroke and recent infections with SARS-CoV2 and Varicella-Zoster virus (VZV). The patient presented with symptoms such as paresthesias, muscular spasms, urinary incontinence, and Lhermitte's sign. Extensive tests were conducted, including blood tests, cerebrospinal fluid analysis, and imaging studies, which showed inflammatory-like lesions in the spinal cord and an acute ischemic stroke in the thalamus. The patient received various treatments, including corticosteroids, plasmapheresis, immunoglobulin therapy, and immunosuppressive therapy, and experienced a gradual improvement in symptoms. The authors suggest that the neurological events may be related to a post-infectious systemic vasculitic process involving both the central nervous system and systemic vessels.
