Common risk factors in the shadow of rare pregnancy complications: Spotlighting the role of cardiovascular health in placental abruption.

- Abstract
The article discusses the relationship between adverse pregnancy outcomes (APOs) and the risk of cardiovascular disease (CVD) in both birthing adults and their offspring. While there is evidence linking commonly occurring APOs, such as hypertensive disorders of pregnancy and gestational diabetes, to a higher risk of CVD, there is limited data on the less commonly occurring APO of placental abruption. The article highlights the need for large epidemiologic datasets to capture both pregnancy-related and CVD-related events and describes the Placental Abruption and Cardiovascular Event Risk (PACER) study, which aims to create a population-based dataset in the United States. The study authors successfully linked data from multiple sources and found that placental abruption was associated with higher rates of perinatal complications. However, the dataset has limitations, including missing data and being limited to one US state. The article also raises questions about the relationship between hypertensive disorders of pregnancy and placental abruption, the influence of placental abruption on long-term blood pressure trajectories, and the potential benefits of tighter blood pressure control during pregnancy. The promotion of cardiovascular health during pregnancy is emphasized as a key strategy for preventing CVD in birthing adults and their offspring.
