Finding of the authors that appeals in denial of medical coverage are rare; Idea that some portion of appeals concerning medical necessity involve the problem of "social uncertainty" regarding the boundaries of medical care; Perception that the essence of health insurance is an enforceable, formal promise to furnish a specified form of coverage to persons who meet contractual qualification standards; Idea that health care consumer protections are a topic of concern to the health care coverage debate.
