- Abstract
Despite evidence of clinical improvement, in our patient, TMS parameters remained unchanged after levodopa administration, unlike PD patients in whom SICI normalizes after medication [[5]]. However, our patient's peculiar clinical features, instrumental findings, disease course, and response to pharmacological therapy do not seem to point to other kinds of monogenic combined dystonia with parkinsonism (e.g., rapid onset dystonia-parkinsonism) [[4]]. Further genetic testing for mutations in DTY genes identified after the first patient's observation (i.e. after 2003) and associated with dystonia-parkinsonism syndromes could not be performed because our patient did not provide his informed consent in clinical follow-up.
