- Abstract
Waldenström macroglobulinaemia (WM) is an uncommon lymphoplasmacytic lymphoma characterised by an IgM monoclonal gammopathy and bone marrow infiltration. Serum protein electrophoresis showed a slightly elevated IgM level (7.24 g/l) and a monoclonal IgM band too weak to quantify. Caution should be taken when initiating rituximab in patients with a high IgM level (>40 g/l) as this can precipitate an IgM flare and lead to a transient rise in IgM levels and an increased risk of hyperviscosity.
