Primary colorectal sarcoma is a extremely rare malignancy (0.1% of all the colon cancer) that is associated with poor patient outcome. Surgery is still preferred treatment for colorectal sarcoma while the use of chemoradiotherapy remain controversial. A 73 year old woman was referred to J. Hospital due to huge myoma with secondary degeneration. Cancer antigen 125 was 2,172 U/mL. Computed tomography (CT) revealed 11 cm irregular mass arised from left ovary and multiple peritoneal seeding. Cytoreductive operation was done. High grade serous carcinoma with stage IIIc. The pelvic lymph node of obturator was metastasis. Taxol/carboplatin/bevacizumab chemotherapy 6 cycle was done. There was still growing tumor after 6th therapy. Positron emission tomography/CT showed 5 cm mass at right upper quadrant area. Secondary cytoreductive surgery was done. T-colon segmentectomy and end to end anastomosis was done. The pathologic report was poorly differentiated colon high grade sarcoma. The refractory tumor was double primary cancer.
