- Abstract
INTRODUÇÃO O estado nutricional inadequado na gestação pode ser causa de comorbidades para o binômio mãe-filho, onerando os cofres públicos, acarretando sequelas que irão comprometer o capital humano do bebê e a qualidade de vida de ambos, este estudo tem como objetivo determinar os fatores associados à obesidade gestacional em mulheres atendidas em unidades básicas de saúde de São Luís-Ma. MÉTODOS Estudo do tipo transversal, realizado com dados secundários da pesquisa intitulada “Estado nutricional de iodo, sódio e potássio entre gestantes, nutrizes e lactentes brasileiros: Um estudo multicêntrico”, que obteve aprovação do Comitê de Ética em Pesquisa (CEP) da Universidade Federal de Viçosa sob o número do parecer consubstanciado 2.496.986, e dos CEP dos demais centros participantes. Foram considerados elegíveis para avaliação as gestantes residentes nas zonas urbanas do município de São Luís e usuárias da rede pública de saúde. Não foram incluídas no estudo mulheres com diagnóstico referido de hipotireoidismo e histórico de doença e/ou cirurgia tireoidiana. Avaliou-se 288 gestantes, de 15-39 anos, a partir das variáveis demográficas (idade, cor da pele e estado civil), socioeconômicas (escolaridade e renda, estilo de vida (consumo de cigarros no período da entrevista)e de saúde (estado nutricional pré)gestacional, estado nutricional por semana gestacional, possuir cartão da gestante, gravidez planejada, trimestre gestacional, número de consultas no pré-natal, número de gestações anteriores, hipertensão arterial anterior à gestação). Ultilizou-se modelo de regressão multivariado de Poisson Robusto para identificação das variáveis de interesse associadas à obesidade gestacional. RESULTADOS A partir do estado nutricional pré-gestacional, segundo o IMC, 55,7% encontrava-se adequado, 25,1% com sobrepeso, 11,4% com obesidade e 7,8% com baixo peso. Em relação ao estado nutricional por semana gestacional, segundo o IMC, 37,2% estava eutrófica, 3,4% com sobrepeso, 17,0% abaixo do peso e 13,4% com obesidade. As variáveis que apresentaram o p-valor <0,10 na análise não ajustada e foram selecionadas para análise ajustada foram: idade, trimestre da gestação, IMC pré-gestacional e tem ou já teve hipertensão gestacional. Na análise ajustada, permaneceu associada com o estado nutricional na gestação, o IMC pré-gestacional (baixo peso RP=5,51; IC: 2,58-7,89; p<0,001/ sobrepeso RP=4,17; IC: 2,97-5,86; p<0,001/ obesidade RP=4,31; IC: 3,06-6,06; p<0,001). CONCLUSÃO Houve associação estatística significante entre o IMC pré-gestacional e a obesidade gestacional, evidenciando que as mulheres que iniciam a gestação com o IMC pré)gestacional de sobrepeso e obesidade apresentam maiores chances de permanecerem obesas na gestação.
