- Abstract
Wstęp W praktyce oddziału intensywnej terapii (OIT) w ciągu ostatnich lat bardzo ważnym zagadnieniem stało się postępowanie w sepsie wirusowej. W obliczu kryzysu pandemicznego mierzyliśmy się z ogromnymi problemami zarówno związanymi z leczeniem pacjentów z COVID-19, jak i nieadekwatnymi do zwiększonych potrzeb środkami dostępnymi na OIT. Pomimo zaawansowania w leczeniu śmiertelność w tej grupie chorych była bardzo wysoka i wynosiła od 35 do 68%. Z tego powodu wydaje się kluczowe, by móc szybko oceniać rokowanie pacjentów przyjmowanych na OIT i podejmować niezbędne decyzje diagnostyczne. W przebiegu sepsy często dochodzi do zaburzeń funkcjonowania jelita. Podczas pracy z pacjentami chorymi na COVID-19 często obserwowano, obok objawów niewydolności oddechowej, nasilone objawy uszkodzenia przewodu pokarmowego - zaparcia, biegunki, wymioty. Coraz większe znaczenie zyskuje hipoteza mówiąca o błędnym kole chorobowym powodowanym nadmierną przepuszczalnością jelit i translokacją bakteryjną, co dodatkowo wpływa na stymulację systemowej reakcji zapalnej. Cel W pracy skupiono się na poszukiwaniu odpowiedniego modelu opartego o pomiar stężenia biomarkerów uszkodzenia jelita, by skutecznie i szybko oceniać ryzyko zgonu. Materiał i metody Do badania kwalifikowano wszystkich pełnoletnich pacjentów septycznych przyjmowanych na OIT z rozpoznaniem COVID-19 w okresie od 2020 do 2021 r., z wykluczeniem osób cierpiących na przewlekłe zapalne choroby jelit. Pod uwagę wzięto wybrane biomarkery uszkodzenia ściany jelita: I-FABP (ang. intestinal fatty acid-binding protein) i cytrulinę. W próbkach krwi mierzono poziom biomarkerów przez 5 kolejnych dni, korelowano ze standardowymi badaniami laboratoryjnymi i oceniano stan kliniczny pacjentów za pomocą skal SOFA, APACHE II i AGI. Wyniki Wyniki analizy statystycznej wskazują, że najskuteczniejszy dla przewidywania ryzyka zgonu pacjenta jest wieloczynnikowy model oparty o poziom I-FABP i parametry kliniczne (skala APACHE II i wystąpienie niewydolności układu krążenia). Wnioski I-FABP, w połączeniu z danymi klinicznymi zawartymi w naszym modelu, jest użytecznym narzędziem do określenia ryzyka zgonu w dobie przyjęcia na OIT. Dodatkowo określenie stopnia zaburzeń czynności jelita na podstawie zmian stężenia biomarkerów może być pomocne w podejmowaniu właściwych decyzji dotyczących żywienia, poszerzenia diagnostyki układu pokarmowego i ewentualnych interwencji chirurgicznych, a w obliczu zarządzania kryzysowego ochroną zdrowia - w priorytetyzacji celów leczenia w OIT.
