- Abstract
A diffuse aneurysm of the infra-renal aorta, iliac and common femoral arteries is described, and seven cases conforming to this prototype are presented. The proximal component of this aneurysm is of particular interest, involving, as it does, the origin of the inferior mesenteric artery and the internal iliacs bilaterally. It constitutes a specific vascular problem which has received but little attention in the surgical literature. Its operative correction exposes the patient to the risk of mesenteric ischemia, and the options for avoiding this are limited. In the event, mesenteric ischemia was not encountered during the management of the seven patients-- five with a specific lesion-- suggesting that the collateral supply to the mesenteric circulation is both extensive and complex.
