- Abstract
Wprowadzenie: Transplantacja macierzystych komórek układu krwiotwórczego (TKK) jest leczeniem z wyboru w wybranych nowotworowych i nienowotworowych chorobach wieku dziecięcego. Obserwujemy ciągłą poprawę wyników TKK, stąd populacja dzieci wyleczonych dzięki TKK stale rośnie i konieczne staje się monitorowanie jej odległych następstw. Cel: Celem pracy była ocena przy użyciu klasyfikacji CTCAE występowania i stopnia nasilenia odległych następstw TKK przeprowadzonej u dzieci i młodzieży w Polsce w latach 1999-2016. Materiał i metody: W ramach Narodowego Programu Zwalczania Chorób Nowotworowych oceniono 140 pacjentów (54 dziewcząt, 86 chłopców) w wieku 1,3-17,6 (mediana 6,8) poddanych TKK, w tym 55 po auto-TKK i 85 po allo-TKK, którzy zostali zarejestrowani w bazie oceniającej odległe następstwa leczenia przeciwnowotworowego u dzieci. Wyniki: Mediana czasu od zakończenia leczenia do momentu oceny wynosiła 6,7 (2,7-13,8) roku. Rozkład pacjentów w zależności od choroby podstawowej prezentował się następująco: ostre białaczki ― 70 (50,0%), chłoniaki ― 14 (10,0%), złośliwe guzy lite ― 52 (37.1%), inne rozpoznania - 4 (2,9%). U 131/140 (93,6%) pacjentów, w tym u 48 (87,3%) po auto-TKK i 83 (97,6%; p = 0.747) po allo-TKK, wystąpiło przynajmniej 1 niekorzystne zdarzenie zdrowotne. Łącznie w całej analizowanej grupie zarejestrowano 643 zdarzenia niepożądane, od 1 do 12 zdarzeń na jednego ozdrowieńca. W całej grupie dominowały powikłania skórne (42,9%), zaburzenia wzrastania (38,6%), zaburzania ze strony układu moczowego (32,9%) oraz zaburzenia hormonalne tarczycy (30,0%). Profil odległych powikłań różnił się w zależności od rodzaju przeprowadzonej transplantacji (p = 0.011). U pacjentów poddanych allo-TKK najczęściej spostrzegano zaburzenia wzrastania (60.0%), powikłania ze strony układu krążenia oraz skórne (po 32,9%), podczas gdy po auto-TKK dominowały powikłania skórne (58,2%), zaburzenia układu moczowego (47,3%) oraz mięśniowo-szkieletowego (38,2%). Wnioski: U niemal wszystkich pacjentów poddanych TKK obserwowano co najmniej jedno, a najczęściej więcej niż jedno odległe następstwo zastosowanego leczenia. Częstość obserwowanych powikłań jest równa po auto- i allo-TKK, jednak profil skutków ubocznych zależy od rodzaju przeprowadzonej transplantacji.
