- Abstract
Introducción. El diagnóstico temprano del SARS-CoV-2 permite intervenciones para evitar la expansión de la enfermedad y reducir el impacto en los sistemas de salud. Los modelos de predicción basados en síntomas pueden mejorar las decisiones clínicas y ser una alternativa para la asignación de recursos. Objetivo. Crear un modelo basado en síntomas para la predicción del resultado positivo por RT-PCR para SARS-CoV-2 usando métodos de epidemiología clásica y el prendizaje automático. Materiales y métodos. Se incluyeron participantes del Proyecto CoVIDA y se hizo una regresión logística utilizando el modelo con mejor criterio de información de Akaike ajustado a la plausibilidad biológica. Se hizo un análisis de sensibilidad utilizando aprendizaje automático con bosques aleatorios, máquina de soporte vectorial y potenciación de gradiente (XG boosting). Resultados. Se incluyeron 58.577 participantes con una positividad del 5,7 %. La regresión logística mostró que la anosmia (aOR=7,76, IC95%: 6,19, 9,73), la fiebre (aOR=4,29, IC95%: 3,07, 6,02), la cefalea (aOR=3,29, IC95%: 1,78, 6,07), la tos seca (aOR=2,96, IC95%: 2,44, 3,58), y la fatiga (aOR=1,93, IC95%: 1,57, 2,93) se asociaron con la infección por SARS-CoV-2. El modelo final tuvo un área bajo la curva de 0,73 y clasificó correctamente al 85 % de los participantes. Conclusiones: Este modelo puede usarse para priorizar la asignación de recursos para el diagnóstico de COVID-19, el aislamiento temprano, y el rastreo de contactos en individuos con alta probabilidad de infección antes de la prueba confirmatoria. Esta estrategia puede ser de gran importancia para la adopción de decisiones en salud pública y la atención clínica en contextos con recursos limitados como Latinoamérica.
