- Abstract
This examination compares the differential treatment of monozygotic (identical) twins with a Class II, division 2 malocclusion, combined with severe anterior crowding in both the maxilla and mandible. Because of a learning disability, the first twin underwent camouflage therapy by extracting the maxillary first premolars, while the second was treated with cervical headgear. For both children, a lingual arch was applied to maintain leeway space, along with interproximal enamel reduction in the lower anterior segment. Total active treatment duration of the headgear patient amounted to 3 years and 4 months (excluding lingual arch treatment and including 22 months wearing headgear), while the extraction patient completed treatment after 2 years. Cephalometric analysis of the headgear patient showed limited anterior development of the A point, a lesser anterior rotation of the mandible and a stronger labial inclination of the maxillary incisors. The sagittal skeletal relation developed from distal to neutral with headgear treatment, whereas following extraction treatment, the distal skeletal relation persisted. Comparison of the facial profiles does not clearly reflect the observed differences. The extraction treatment proved to be the more effective strategy to relieve crowding in the maxilla and achieve good occlusion.
