- Abstract
Os sedativos alfa-2 agonistas, como a detomidina, são amplamente utilizados na medicina veterinária e os seus efeitos de sedação devem ser acompanhados ao longo do tempo de manipulação do animal, na intenção de controlar o efeito medicamentoso no seu uso clínico. Alguns métodos para avaliar a profundidade e a qualidade da sedação são utilizados, sendo eventualmente subjetivos, baseados na experiência do observador. Neste sentido, a expressão facial já foi incorporada em escalas descritivas de avaliação sedativa e a FaceSed é uma escala já validada, que não envolve a manipulação do cavalo e permite que seja acompanhado o estado de sedação do animal. A FaceSed foi adaptada e modificada a partir das unidades de ação facial da escala de avaliação de dor em equinos (HGS), onde avaliam-se a posição de orelhas, abertura orbitária e relaxamento dos lábios inferior e superior, com três pontuações para cada uma das quatro unidades de ação facial, baseadas na expressão esperada de relaxamento muscular em ausente (0), parcialmente sedado (1) ou obviamente sedado (2), podendo ser feita, por fim, a somatória destes valores. O objetivo deste estudo foi avaliar o tempo do efeito sedativo da aplicação de detomidina identificável pela utilização da escala FaceSed. Foram utilizadas quatro éguas pertencentes ao plantel didático da Fazenda Experimental Gralha Azul da PUCPR, hígidas ao exame clínico e conduzidas ao tronco de contenção onde realizou-se a coleta inicial (T0) de vídeos de 30 segundos de cada animal, focando a região lateral da face. Na sequência, dois dos animais ficaram no tronco de contenção sem a sedação (grupo controle, GC) e os outros dois foram sedados com cloridrato de detomidina na dose de 10 µg/kg IV (Dormiun, Laboratório Agener União) (grupo detomidina, GD). Foram novamente gravados vídeos aos 5 (T5), 30 (T30) e 90 (T90) minutos pós-sedação, dos quais foram extraídos frames para que fosse realizada a avaliação pela FaceSed por três avaliadores treinados na utilização da HGS. Foram utilizadas três fotos de cada animal em cada tempo, com edição para retirada de fundo e padronização do posicionamento da cabeça (Keynote, versão12.0, Apple Inc.), ordenadas aleatoriamente. Os dados foram analisados utilizando o programa SPSS Statistics (IBM). A alta consistência interna (Cronbach a = 0,96) indicou uma boa intercorrelação entre os itens. A correlação intraclasse foi alta (0,87; p < 0,001) e na avaliação estatística de item-total, as correlações mais altas foram obtidas, respectivamente, na abertura orbitária, relaxamento de lábio inferior, posição de orelhas e relaxamento de lábio superior. Na avaliação do Kappa para correspondência à atribuição da condição de sedado ou não pelos avaliadores, obteve-se maior consistência (0,76) ao considerar apenas T5 e T30. Quando incluído o T90, o valor de Kappa reduziu para 0,65, representando menor consistência avaliativa. Esta diferença no resultado pode ser atribuída à dose/tempo de atuação do fármaco, sendo que no T90 os animais já se encontravam recuperados da condição sedativa. O comportamento dócil dos animais pode ter reduzido a especificidade, além de que a avaliação estática, por fotos, pode limitar a observação da dinâmica do comportamento animal, reduzindo também a capacidade de identificação do efeito sedativo.
