- Abstract
This letter is a response to a review by Li et al. on the microstructural and hemodynamic changes that occur after pars plana vitrectomy for different vitreoretinal diseases. The authors summarize the vascular modifications found after vitreous surgery, including specific quantitative parameters measured with optical coherence tomography (OCT) angiography and structural OCT. They also discuss the post-vitrectomy dynamic changes related to increased oxygenation within the vitreous cavity, which can lead to hemodynamic events such as arteriole constriction and improved visual function. However, the authors note that some of the structural changes observed may be related to the underlying diseases rather than solely to vitrectomy. They suggest that biomarkers from other functional studies, such as the dynamic vessel analyzer, may provide more reliable information about retinal hemodynamic changes. Overall, while the review is methodologically accurate, the role of vitreous removal in producing hemodynamic changes remains somewhat uncertain and difficult to evaluate.
