Topical Phosphodiesterase-5 Inhibitor Improves Wound Healing of Irradiated Skin.

- Abstract
Background: Wound healing is significantly delayed in irradiated skin. Localised radiation skin injury account for the vast majority of accidental radiation exposures and clinically from cancer radiotherapy. Since different protein mediators are well-established players in wound healing, emerging evidence indicates that nitric oxide (NO) plays a key role in ensuring appropriate intracellular communication during wound repair. In this context, Sildenafil Citrate, a PDE-5 inhibitor is used to investigate its effect on radiation-induced wounds. Objective: To study the efficacy of topical Sildenafil citrate hydrogel (SCH) in the management of x-ray induced skin injury. Methods: The dorsal flank skin of Sprague dawley rats were irradiated with the single dose of 45Gy x-radiation. Acute skin damage and wound healing were evaluated by ulceration contraction, clinical skin grading, histology and SEM imaging. Further, antioxidants assays and hydroxyproline content were evaluated. Statistical tests and post-hoc comparisons were performed. Results: In vivo, repeated dermal SCH treatment for 28 days significant (p<0.05) reduced the size of the radiation wound as compared to control [F11,300=974.420]. Although the skin damage score remained similar on individual days, SCH treatment resulted in less damage overall (% difference=71%). SCH opposes damage by significantly raising NO levels as compared to control. Further, the wound recovery is represented by tissue histology and SEM images. SCH also significantly (p<0.05) reduced localized skin oxidative stress. Conclusion: SCH reduces acute skin radiation damage in rats.
