This document is a reply to a previous clinical opinion article on the maternal-fetal interface. The authors discuss the concept of evolutionary conflict and cooperation between mother and fetus. They argue that while there are trade-offs and divergent interests between the two, most uncomplicated pregnancies are the result of cooperation. They also address the persistence of obstetric syndromes and the challenges of ensuring optimal survival for both mother and fetus.
