- Title
It is not always your fault, but how do you prove it?
- Authors
Foss, Tata D.
- Abstract
The author comments on the role of a hospital's management in ensuring patient safety. The author reveals how the practice of suspending National Health Service staff involved in patient safety issues cost the service over £ 40 million annually. She mentions an Incident Decision Tree developed by the British National Patient Safety Agency. She suggests that hospital management should check out their systems first and complete the tree to determine the root cause of patient safety issues that arise.
