- Abstract
The purpose of this study was to conduct a systematic review of studies that have evaluated the results of preoperative chemoradiotheraphy (CR) for advanced cancer of the cervix in terms of overall survival (OS), disease-free survival (DFS), risk factors for recurrence and surgical morbidity. Retrospective survey was carried out through systematic papers published between 1997 and 2009. In total, 182 articles were found. After the refinement of the search by title words, 17 articles meet the criteria for inclusion are proposed. Regarding to overall survival in 3 and 5 years for the series based on CR treatment schemes as single therapeutic modality we were able to identify an OS average of 66% and 73%, respectively. These results correlate positively to those studies in which the CR was applied with a neoadjuvant character, which presented na OS average of 75.2% and 74% for 3 and 5 years. Similar results were also observed when we compared the data on the average DFS for a 5-year follow-up for the two groups. In the group treated with exclusive CR, survival was 67% in 5 years. For the neoadjuvant CR group the DFS in 5 years was 74% of the cases. In all studies, positive linfonodal status and the presence of macroscopic residual disease configured as the main risk factors for recurrence. However, based on this work, it is not possible to say that there is a clear benefit of preoperative CR for locally advanced cervical cancer, although there is some evidence that in selected cases, patients could be benefited from this treatment. New randomized studies are required to establish the role of each therapeutic modality, its applicability and expected results.
