FACTORES CLÍNICOS Y ELECTROFISIOLÓGICOS ASOCIADOS A POBRE PRONOSTICO FUNCIONAL A LARGO PLAZO EN PACIENTES CON SÍNDROME DE GUILLAÍN-BARRE.

- Abstract
Introducción: Existen factores de riesgo de pobre pronostico funcional en pacientes con síndrome de Guillain Barre (SGB) a 6 meses (recuperación de la marcha independiente), sin embargo, en población mexicana no existe información. Objetivo: analizar factores de riesgo de pobre pronostico funcional a 6 meses en pacientes con SGB en población mexicana. Registro: 145/22 Material y métodos: Estudio observacional de una cohorte ambispectiva de pacientes con síndrome de Guillain-Barré (SGB), de un solo centro médico durante el periodo comprendido entre el 1 de enero de 2018 y el 31 de diciembre de 2022. Se incluyeron en el estudio a aquellos pacientes que tuvieron seguimiento clínico durante un período de 6 meses después del diagnóstico inicial. Obtuvimos información de variables clínicas y paraclínicas. A los estudios de neuroconducción aplicamos los criterios de Hadden para clasificar en variantes; definimos daño axonal severo de nervio motor al PAMC distal =1.0mv. Consideramos pobre pronostico funcional a 6 meses a la No recuperación de la marcha independiente. A través de regresión logística binaria, analizamos factores clínicos y electrofisiológicos de riesgo de pobre pronostico funcional. Resultados: Se incluyeron un total de 282 pacientes en el estudio, de los cuales el 72.3% eran hombres, con una edad promedio de 46 ± 16.2 años. El 37.6% de los pacientes tenía antecedentes de diarrea, con un puntaje promedio en la escala MRC de 33.2 ± 17.3 puntos. El puntaje promedio en la escala EGOSm al ingreso fue de 6 (RIQ 3-7) puntos. Un 31.6% de los pacientes requirió ventilación mecánica invasiva (VMI). Las variantes más frecuentes del SGB fueron la polineuritis inflamatoria aguda (AIDP) en un 43% de los casosy la variante Axonal en un 39%. Presentaron pobre pronostico funcional 59 (21%) pacientes, presentando: antecedente de diarrea (51% vs 34%,p=0.023), puntuación en la escala MRC score (17.2±13.5 vs 37.4±15.7, p=<0.001), puntuación en escala EGOSm [7 (RIQ 6-8) vs 5 (RIQ 2-7)], p=<0.001), requerimiento de VMI (68% vs 22%), variante axonal (62.5% vs 42%, p=0.006), daño axonal severo en nervio mediano (59% vs 30%, p=<0.001), cubital (61% vs 28.6%), tibial (61% vs 36%), p=0.001), y peroneo (70% vs 39%, p=<0.001). A través de modelo multivariable, son de riesgo independiente para pobre pronostico funcional los siguientes factores: puntuación mayor en escala EGOSm OR 1.5(1.2-1.9), p=<0.001; requerimiento de VMI OR 3.6 (IC95% 1.7-7.7), p=001; daño axonal severo (PAMC =1.0mV) de nervio peroneo OR 2.4(1.0-5.4), p=0.031. El rendimiento de este modelo a través de AUC=0.84, (IC95% 0.789-0.892), p=<0.001. Conclusiones: El daño axonal severo de nervio peroneo y la necesidad de ventilación mecánica invasiva son factores de riesgo independientes a escala EGOSm para pobre pronostico funcional a 6 meses en población mexicana. Estos hallazgos pueden ser de utilidad en la identificación temprana de pacientes con mayor riesgo de mal pronóstico, lo que permitiría una intervención clínica más precisa y oportuna.
