The Effectiveness of Antimicrobial Agents against Acanthamoeba Cysts Isolated from Clinical Specimens.

- Abstract
Introduction: Acanthamoeba keratitis is a chronic corneal infection which can cause blindness if left untreated. Objective: This study was performed to evaluate the effectiveness of the antimicrobial agents against Acanthamoeba cysts isolated from clinical specimens at therapeutic doses. The minimum cysticidal concentrations (MCC) of the antimicrobial agents that were effective against the cysts were determined. Materials and methods: The antimicrobial agents used were 0.1% propamidine isethionate, 0.02% chlorhexidine, 0.5% gentamicin sulphate and 10% povidone iodine. The cyst suspension was tested with the antimicrobial agents which had been diluted in the microtiter plate. After a 24 hour incubation, the microtiter plate was rinsed with Page Amebic Saline (PAS) into the vials. The vials then were centrifuged. The cysts (deposits) were cultured on the non-nutrient agar (NNA) seeded with E. coli. After a 48 hour incubation, the plates were observed under the microscope for the presence of trophozoites. The observation was continued for 14 days. Results: Chlorhexidine showed the lowest median MCC at 25 (6.25-25) µg/mL, followed by propamidine isethionate with 156.25 (31.25-500) µg/mL. Gentamicin sulphate demonstrated higher median MCC at 3750 (2500-5000) µg/mL. The highest median MCC was povidone iodine at 12500 (3125-12500) µg/mL. Conclusion: Chlorhexidine is the best cysticidal agent and the combination therapy with propamidine isethionate is recommended to produce a more effective effect.
