Discharge decisions.
Abstract
This article presents results of the report, Leaving Hospital: Revisited, published by the British Commission for Social Care Inspection. The report found that very few people who go into residential care at the time of discharge return to their own homes, and that older people are less likely to need residential care if the right support and rehabilitation is offered to them on leaving hospital.
