- Abstract
Introduction: This video presentation introduces the first case of laparoscopic radical cystoprostatectomy with extracorporeal ileal conduit for the management of muscle invasive bladder tumor in VietNam. Methods: This procedure was performed through transperitoneal approach with 5 in Trendelenburg position. The ureters were determined and dissected at UVJ. The peritoneum was incised anterior to the bladder. The deep dorsal veins were controlled by Harmonic Scalpel and the urethral stumps were prepared. The posterior bladder was separated till reaching Denonviller's fascia and incised. Then the lateral bladder was free from the pelvic wall. The lateral pedicles of the bladder were. The prostatic apex and neurovascular bundle was spared. The mass was completely free. Bilateral iliac lymphadenectomy was then performed. A 10 cm subumbilical skin incision is done and ileal neobladder with Studer technique was performed. Uretero-ileal anastomosis was done. The pouch was introduced to the abdominal cavity and the urethra-ileal anastomosis was completed by hand-assisted laparoscopy. Results: Surgical margin was negative. The operation time was 7hours. Blood loss was less than 100 ml. The time to oral intake was 4 day and discharged after 7 day. All catheters were removed after two weeks. There were no perioperative morbidities. The patient had the daytime continence but nighttime was incontinence after 2 months. The neobladder capacity was 420 ml, residual urine was 20 ml. Nocturnal spontaneous erection was recognized. Conclusions: Laparoscopic radical cystoprostatectomy with extra-corporeal ileal conduit is a hard labor procedure but feasible. This was only our first experience.
