Opportunistic Salpingectomy in Premenopausal Women for Prevention of Ovarian Cancer.

- Abstract
Introduction: Epithelial ovarian cancers (EOCs) are the most common cause of death from gynecological malignancy. There has been increasing evidence that fallopian tubes are considered the most important site of origin of pelvic high grade serous carcinoma. Salpingectomy is thought to be effective in reducing ovarian cancer risk and prolonging average life expectancy, especially in high risk women. Removing fallopian tubes is not a difficult procedure and is well within the skill sets of gynecologic surgeons. Objective: The objective of this study was to assess the ovarian function in patients before and after opportunistic salpingectomy with hysterectomy for benign uterine pathologies. Methods: 30 patients were recruited and underwent total or sub-total hysterectomy (abdominal or laparoscopic) plus bilateral salpingectomy. Ovarian reserve modification was tested by the difference between e-operative and 3 months' postoperative values of Antral Follicle Count (AFC), Anti-Müllerian Hormone (AMH) and Follicle Stimulating Hormone (FSH) and was recorded for each patient. For each case, operative time and variation of hemoglobin (Hb) level were also recorded. Results: There were no statistical differences between the e-operative and the 3 months' postoperative values of AFC, AMH and FSH, as well as operative time and Hb level. Conclusions: The addition of bilateral salpingectomy to hysterectomy for prevention of ovarian cancer do not adversely affect ovarian function. In addition, opportunistic salpingectomy does not increase operative time or blood loss. Therefore, if adnexectomy is not planned with hysterectomy, at least salpingectomy should be performed to event pelvic high grade serous carcinomas.
