- Abstract
Introduction: Acute otitis media is a common condition in childhood that can evolve with a rare complication called Gadenigo Syndrome, characterized by the triad: complicated otitis media with purulent otorrhea, pain in the area of trigeminal nerve innervation and abducens nerve palsy. It is related to the involvement of the petrous apex of the temporal bone. Objective: To report a case of a rare complication of otitis media with petrositis. Report: A 8 years old child, referred to neurological emergency with complaint of intense headache in right hemiface with diplopia. Associated with a history of redicivating otitis, in which the last episode was 1 month ago. Physical examination revealed strabismus with limited abduction of the right eye, without facial paralysis. Otoscopy of the right ear: bulging tympanic membrane with retrotympanic secretion. CT evidenced filling of the right mastoid. Laboratory exams and liquoric puncture without alterations. MRI showed signs of petrous apicitis with osteomyelitis of the skull base to the right and formation of a small local abscess with cranial nerves involvement. A broad spectrum intravenous antibiotic and corticotherapy were instituted. An emergency otorhinolaryngological evaluation was requested in our Service, where the patient was diagnosed with acute otitis media complicated by petrositis, surgical treatment was chosen: myringotomy with insertion of a ventilation tube and mastoidectomy. At follow-up, the patient evolved with no pain complaints and eye movements recovered. Conclusions: Show the importance of the diagnosis and joint approach in the recognition and treatment to reduce morbidity and mortality of this complication.
