- Abstract
The article discusses a recent guidance statement on colorectal cancer (CRC) screening in asymptomatic average-risk adults. The authors express confusion over the recommendation of biennial fecal occult blood testing (FOBT) and FIT in the guidance statement, as evidence from a US clinical trial shows that annual high-sensitivity FOBT reduces CRC mortality by 33% at 18 years of follow-up. The authors argue that the biennial screening group had lower reductions in CRC mortality. They also mention real-world evidence showing a reduction in CRC incidence and mortality with an annual FIT-based screening program in the US. The authors suggest that the guidance statement creates confusion and ignores direct evidence from target populations, as well as deviating from the current standard of annual FOBT/FIT. They highlight that the current quality measure allows up to a 23-month gap between tests, while the recommendations in the guidance statement would allow gaps of up to 35 months.
