La mobilizzazione precoce e la prevenzione della Intensive Care Unit Acquired Weakness (ICU-AW): una revisione narrativa della letteratura.

- Abstract
Introduction: The Intensive Care Unit Acquired Weakness (ICUAW) is a complication that can occur in a high percentage of patients admitted to the intensive care unit, characterized by fatigue and neuromuscular weakness, and may persist even years after discharge. Early mobilization interventions on critical patients, including during mechanical ventilation, have shown positive and encouraging results in preventing ICUAW. Objective: The main purpose of this literature review is to describe the safety, the feasibility and the effectiveness of early mobilization activities in the prevention of ICUAW. Materials and Methods: a literature review was performed, searching in several biomedical databases (Pubmed, Cochrane library, UpToDate, CINAHL - Cumulative Index to Nursing & Allied Health Database), with a time limit of 15 years. The clinical query combined the keywords "ICUAW", Early Mobilization", "Adult", "Intensive care unit", "Critical Illness", "ICUAW". Results: The research revealed 236 studies; after removing the duplicates, applying the search filters and after reading the title, abstract and full text, a total of 16 studies were considered (5 systematic reviews, 4 randomized controlled trials, 6 reviews and 1 clinical trial). Conclusions: Early mobilization in the ICU was found to be a safe and feasible practice, and increased functional abilities of the patients, who showed significantly higher Barthel Index scores and were able to perform more activities such as sitting on the bed, moving into an armchair until walking. Further studies would be appropriate, with uniform criteria for assessing outcomes, in order to be able to evaluate the effectiveness of these interventions.
