From NAFLD to MASLD: Promise and pitfalls of a new definition.

- Abstract
This article explores the proposal to change the name of non-alcoholic fatty liver disease (NAFLD) to metabolic dysfunction-associated steatotic liver disease (MASLD). The authors discuss the potential benefits of this change, such as reducing stigma and highlighting the role of insulin resistance in the disease. However, they also raise concerns about the definition of MASLD and its clinical implications, particularly regarding the high prevalence of cardiometabolic risk factors in the general population and the discordance between these risk factors and insulin resistance. The authors recommend further research to validate the proposed definition and suggest classifying individuals without cardiometabolic risk factors as having "early" MASLD. They caution against mislabeling steatosis linked to insulin resistance as "cryptogenic steatosis" and stress the importance of lifestyle changes to prevent liver disease progression. The decision to change the definition of MASLD was supported by a slim majority, and the authors suggest that further debate is needed to reconsider the relationship between cardiometabolic risk factors and steatosis.
